• your address for treatment at your home.

We use the information that you have given us in order to:

• locate your home for treatment at your home,
• safely and thoroughly assess and treat you with physiotherapy.

• your GP, NHS or the Emergency Services,

How we store your personal information

Your information is securely stored within a secure ICT reservoir. Handwritten notes are stored in a secure filing unit.

We keep contact details and medical records for 6 years after last entry or 3 years after death. We will then dispose your information by deletion from ICO platforms and physical shredding of paperwork.
